  1. Download the claim form from the ERGO company website.
  2. Complete the form fully and attach the required supporting documents as specified, including your mobile phone number and email address for claim progress notifications.
  3. You can submit your claim documents to ERGO Insurance via 3 channels:
    • Post: Accident and Health Insurance Claims Team, ERGO Insurance (Thailand) Public Company Limited, 126/2 Krung Thon Buri Road, Bang Lamphu Lang, Khlong San, Bangkok 10600
    • Email: Accident and Health Insurance Claims Team, ERGO Insurance (Thailand) Public Company Limited, Taclaim@ergo.co.th (for claims that do not require original documents/receipts only)
    • Agent/Broker from whom you purchased the policy
  4. Once ERGO Insurance receives your claim documents, a confirmation email will be sent to the email address provided in the form.

  • If the claim information/documents are complete, ERGO Insurance will process the claim within 15 days and notify you of the result.
  • If the claim information/documents are incomplete, ERGO Insurance will contact you to request the missing information/documents within 5 business days after receiving the claim notification.
  • For claims exceeding THB 100,000, identity verification is required by submitting a photo of the insured or beneficiary holding their national ID card or passport with the face clearly visible. Please redact religion and blood type information (if any), and include it with the claim documents.

Documents required for all coverage agreements

  • Claim form as specified by the company
  • Copy of passport with certified true copy signature, and travel evidence showing entry/exit stamps issued by the Immigration Bureau (IB). If entry/exit was made through an automated system, please submit a Flight Itinerary or E-Ticket showing the travel dates instead.
  • Copy of the insured's national ID card with certified true copy signature

Documents specific to the benefit category the insured wishes to claim

  • Refer to the claim form on the ERGO company website, or
  • Document list as specified in the insurance policy for each coverage
